CareAscension is a digital management platform designed to reduce barriers to shelter access by improving coordination between street outreach teams, shelters, healthcare providers, and city agencies. The system provides real-time shelter bed tracking and coordinated placement tools, enabling navigators and case managers to quickly identify available beds and connect individuals experiencing homelessness with safe shelter and support services.

The project addresses a growing challenge across the United States, where more than 771,000 individuals experience homelessness on any given night. In Arizona alone, homelessness has increased significantly in recent years, with more than 14,000 individuals experiencing homelessness in 2023. One of the key drivers of this crisis is the lack of centralized systems connecting shelters, outreach teams, and healthcare providers, which leads to fragmented services, delays in placements, and worsening health outcomes.

CareAscension introduces a shared digital infrastructure that allows participating shelters to update bed availability in real time while enabling outreach navigators to immediately see openings across the network. By improving transparency, reducing placement delays, and generating system-level data insights, the platform helps cities better coordinate shelter capacity and improve continuity of care for vulnerable populations.

The platform will launch as a pilot program in Phoenix, Arizona in 2026, working with partners including the City of Phoenix, the Phoenix Navigation Campus, Washington Street Shelter, and Safe Outdoor Space to demonstrate how technology can strengthen coordinated care systems and improve access to shelter across the region.
